Weight loss on GLP-1 is downstream of appetite suppression — and appetite changes much faster than the scale does. A simple 1–5 score recorded at each meal catches the earliest signs of dose tolerance or unexpected appetite return, weeks before the weight curve flattens.

Week 0 (pre-drug): average 3.5–4.5 (normal hunger).
Week 1 of new dose: drops to 2.0–2.5 (strong suppression, sometimes unpleasantly low).

What "tolerance" looks like

What "rebound appetite" looks like
Sudden appetite return — over days, not weeks — usually indicates something specific:
Missed dose past the rescue window. Appetite climbs 24–48 hours after the would-have-been injection.

Weekly avg >3.5: appetite climbing. Advance on schedule if titration ladder allows, or evaluate why the current dose isn't holding.
Should I hold the dose?
Weekly avg ≤1.5 (too suppressed): hold current dose; you may be over-suppressed and risk under-eating.
Weight loss exceeding 2 lb/week on average: hold or even step down to slow the pace.
The food-relationship signal
Appetite scoring also catches the qualitative shift in food relationship that GLP-1 produces:
Pre-drug: meal anticipation is a meaningful part of the day, even pleasant.
Effective dose: meals become more functional; pleasure from food doesn't disappear but is dampened.
Tolerance creeping in: meal anticipation returns; specific food cravings return.
Off-drug: full return of appetite, often briefly amplified compared to pre-drug baseline.
Tracking the score makes these phase transitions visible.
What the score doesn't catch
Reward-driven eating (hedonic). Some users have suppressed hunger but persistent food-reward seeking. The score doesn't distinguish.
Late-evening cravings. Mealtime scoring misses cravings that come at non-meal times.
Emotional eating. Stress, boredom, social cues — these can produce eating despite low appetite scores.
The score is one signal in a richer picture. Combine with weight trend, meal protein, and overall well-being.
Practical implementation
Set a reminder for each meal time. 30 seconds of scoring per meal.
Score before eating, not after. Post-meal "I'm too full to eat more" is a different signal than pre-meal hunger.
Track for at least 8 weeks to see the curve.
Review weekly and tag any score that's very high or very low for explanation.
Use the trend in dose decisions alongside weight and side-effect data.

What if I forget some meal scores?
Gaps are fine. The weekly average is still meaningful with 60–80% of meals scored. Don't backfill from memory.
Does appetite score correlate with weight loss?
Loosely. Lower appetite scores correlate with more weight loss on average, but individual variability is high. Use it as one signal, not the only one.
Should I score by hunger or fullness?
Pre-meal hunger is cleaner. Post-meal fullness is harder to standardize because it depends on what you ate. Stick with pre-meal hunger.
Is appetite return after months a sign to switch drugs?
Possibly. Persistent return at maximum dose can mean it's time to try a different drug class (semaglutide → tirzepatide, or add cagrilintide if/when available). Discuss with the prescriber.
